Gov. Red or Orange Zone Self-Certification March 2021: Download PDF

Self-certification form for March 2021 is available on the website of the Ministry of Home Affairs (Editable PDF here) Must be used for travel for proven work needs or for health reasons.

Various information to be provided at the control event: to declare under one’s own responsibility to be aware of the regulatory measures to control the epidemic from Govt-19, regarding the limitations on the possibility of moving natural persons within national boundaries; Be aware of other actions and limitations issued by orders or other administrative measures approved by the President or Mayor of the region under current regulations; Be aware of the sanctions provided by art. 25 March 2020 Ordinance 4, n. 19, and by art. 16 May 2020 Ordinance 2, n. 33.

It should also be noted that this measure is determined by: Proven work requirements; Health causes; Other reasons acknowledged by the prevailing regulations or the aforesaid orders, directives and other measures defining measures to prevent the spread of infection.
